Gathering Information

After scheduling, you’ll receive secure electronic intake forms through your TherapyNotes client portal, a HIPAA-protected system used for all clinical documentation and communication. You’ll receive an email notification when your forms are ready, and intake is completed directly within the client portal, not via email.

These forms allow me to begin forming your clinical chart and gain a clearer understanding of your background, concerns, and goals.

Completing this information ahead of time helps us use session time intentionally and ensures our work begins with clarity rather than paperwork. You don’t need to overthink your responses. This is simply a starting point, not a test.

If you have questions about the intake process or run into any issues completing forms, you’re welcome to reach out prior to your session.

What You’ll Be Asked to Complete

You may be asked to provide:

  • Basic contact and demographic information

  • Information about your current concerns and what led you to seek therapy

  • Relevant mental health, medical, and medication history

  • Therapy history and areas you would like support with

  • Required consent and practice forms

I only ask for information that helps me provide ethical, thoughtful care.

Your Choice and Safety

You are always in control of what you choose to share and when. If there are questions you’re not ready to answer, it’s okay to leave them blank for now.

That said, your safety is always a priority. Sharing what you’re able to helps me support you thoughtfully and responsibly, and we can continue to build from there together.

Privacy and Security

All intake forms are completed through a secure, HIPAA-compliant system. Your information is confidential and becomes part of your protected clinical record.

A Note About Getting It “Right”

There are no perfect answers. You don’t need to have everything figured out or know how to word things exactly right. You’re welcome to skip questions you’re unsure about or revisit them later.

We can clarify, update, or expand on anything together over time.

Why This Information Matters

Completing intake forms ahead of time allows me to prepare for our first session with intention rather than spending valuable session time on logistics. It helps me understand what brings you in and how to best support you from the start.

This information gives us a clearer foundation so we can focus on what matters most to you.

Time Commitment

Most clients complete the intake forms in approximately 35-45 minutes. You’re welcome to pause and return to them if needed.

What Happens After You Submit

Once your forms are submitted, I review them prior to our first session. We’ll talk through anything that feels important, unclear, or incomplete when we meet.
